In the USA, the President is both the head of state and the head of government. In a constitutional monarchy, such as Canada, the Queen, when she is actually in Canada, and otherwise the Queen's representative, the Governor General, is the head of state. The position is generally a ceremonial one. It is a non-political position. The office holder is deemed to be "above politics." In Canada, the prime minister is the head of government.
The mian kinds are Absolute monarchy and Constitutional Monarchy. With an absolute monarchy, there is one spreme ruler who controls all of the land and the people with no higer form of government above with. With a constitutional monarchy, There is a ruler who is the leader of a government but does not have supreme power. (does not control everything but is still a part of the government)
The term "Imperial President" is a mildly derogatory term. Imperial usually refers to Britain, which is a Monarchy, albeit a Constitutional Monarchy. An "Imperial President" is one who governs like a Monarch, rather than as a commoner. Imperial Presidents often consider themselves as being "above the law" and as a result they sometimes commit breaches of the law. The prime example of this is Richard Nixon.

No one in the government ranks above the president. I suppose you could say he reports to the US people. Congress has the power to remove him from office if strays too far from proper conduct of his office.
