Yes, 40mg of Strattera is a lot to start out with because you're body may not be able to handle that big of a dosage. I'm no doctor, but I've been on Strattera now for a week and my dosage is 18mg, and it makes me feel ugh! If 18mg is a lot, then of course 40mg is a lot to start out with. I hope this somewhat makes sense and helps. Its one of the stronger amounts but I don't think its A LOT. Im starting at 40 and tomorrow I start taking 80 so clearly it cant be too bad if im doubling it

So, say there is a patient who weighs 180lbs (~90kg) with a broken leg, I would start them off on 10--15mg of morphine via an intramuscular injection. After 15--20 minutes, if the patient was still in excessive pain, I might give another 10mg. I'd repeat this, if necessary, up to maybe 50mg, or possibly 60mg. One would NOT start out giving 60mg. It may not kill your patient outright, but it could cause excessive sedation and respiratory depression.
