Why does the AED fail at times?
AEDs, or automatic external defibrillators, are designed to pick up certain life-threatening rhythms such as VF (ventricular fibrillation) and once detected to shock (by direct current) the rhythm back into a more stable rhythm, ideally sinus rhythm. In hospitals the defibrillators they use can shock people, but they can also perform cardioversion. This is where they use synchronised direct current (synchronised with particular parts of the rhythm; R wave, T wave etc) to convert rapid paced rhythms back to normal rhythms.

Defibrillation is the definitive treatment for the life-threatening cardiac arrhythmias, ventricular fibrillation and ventricular tachycardia. Defibrillation consists of delivering a therapeutic dose of electrical energy to the affected heart with a device called a defibrillator. This depolarizes a critical mass of the heart muscle, terminates the arrhythmia, and allows normal sinus rhythm to be reestablished by the body's natural pacemaker, in the sinoatrial node of the heart. Defibrillators can be external, transvenous, or implanted, depending on the type of device used. The AED will give loud and clear instructions on what to do.
