Typically, a negative ion is formed by gaining electrons. This is because electrons hvae a negative charge. An example of a case where a positive ion can be formed by gaining an electron is when an ion with a charge of +2 can gain 1 electron and become +1. Technically in this case, a positive ion was formed when an electron was gained but the trend was that the ion became more negative not positive.

When you say a positive ion, I assume you mean, for example, Fe2+ which means that the atom has lost 2 electrons. The basic rule is: Metals like to lose electrons (and therefore become positive ions, or cations as they are called) and non-metals like to gain electrons (therefore becoming negative ions, or anions).

The way I remember this is:

Onions are bad ----> Anions are negative e.g. Cl-
Cats are good ----> Cations are positive e.g. Ag+

A atom that has gained a electron is a negative ion because electrons are negative and protons are positive. Before the atom was balanced and equal but now there is more negative power if you get what i mean. Hope I helped =D
