Record the make, model, and serial number. Leave the gun at home. Go to your local police station and ask them how the gun should be registered to you. Give them the information. They will likely tell you to bring in the gun. Ask them what condition the gun should be in for you to transport it to the station (locked in case, disassembled, etc). Follow their instructions. You will likely be ok and not get arrested. No guarantees. Some Pa jurisdictions have ambitious DA's.

You have not said how far it has been disassembled, or what model. Put in a bag, take it to a gunsmith. It will NOT be the first gun-in-a-bag he will have seen.
