It would depend on your flight arrangement, but usually the airports are organised so that you can get to the connecting flight w/o having to go through passport control. And as long as that is the case you haven't really entered the US, so you don't need a visa.

From Charleston or Columbia you will have to fly first to Charlotte or Atlanta and then change for a direct flight to Montego Bay. From Charleston I get up at 4:30 in the morning, go to the airport and fly to Atlanta for a 3 hour layover and then land at Montego Bay around 2 in the afternoon, So I spend nearly eight hours in transit just to get to Montego Bay.
