As a medical transcriptionist of some 30 years, I have always changed the dictator's (usually a doctor) verbiage. Example: Dictator: The patient was woken up from general anesthesia. Typed: The patient was awakened from general anesthesia. Never had a report been returned to me as incorrect because of this change.
