That decision is between you and your vet. If the dog can be treated and given some quality of life still, then give your dog a chance. If you and your vet believe he cannot be treated or will not have a good quality of life even with treatment, you may have to consider the option of putting your dog to sleep.
Its entirely your decision. No-one else can (or should) make the choice for you.
