Definitely not! Even though I haven't played minecraft, I'm pretty this method would work for every game out there. Download a VPN Client such as LogMeIn Hamachi (https://secure.logmein.com/products/hamachi/download.aspx)and install it. Both ends, the hoster and the connecter need hamachi. Now create a group and add your friend to it. Once that is done, right click on his/her name and hit "Copy Address". This IP can be used to connect to his/her server without port forwarding. Good Luck.

You do not need a Minecraft account to set up a server, so unlimited. However most computers can only run one server at a time
There's no way to log into a minecraft server as a guest. You have to purchase the game to be able to use minecraft online "legally". The only way you can play with a non-purchased account is by illegally playing on a cracked minecraft client. Which is strictly unreccomended.
