I think it is a test in abdominal examination in which you ask the patient to lift their head (this will contract the abdominal muscles) and check for any evidence of hernia.
It is advisable before this particular examination, you ask the patient to cough first to check for hernia.
Common sense, make sure the groin area is exposed, as by not doing so pretty much would result in failure of your exam.
