The "Discover It" credit card from Discover doesn't charge any fees like annual fee, overlimit fee or foreign transaction fee. However, they still charge 3% fee for each balance transferred.
As I understand, their must be a prompt at the begining of the transaction on the interac machine for you to accept the additional charge. A vendor cannot simply add the user fee to the total of the transaction amount.

A 'foreign card transaction' - is any transaction on your card that has taken place outside your home country. For example - say you live in the UK, and you go to Spain, using your card while you're there... Any purchases or cash advances made on your card would show up as a 'foreign card transaction'.

Payment processing fees depend on the type of transaction. If you are looking at an ACH or electronic check transaction they fee should range from $0.12 - $0.50 depending on the insurance company. If the transaction is a credit or signature debit card it would depend on is the card was swiped, entered manually or acceptable as an online transaction. The additional factors for credit card fees would be the type of card (Visa/MC/AMEX) and then is the card has a rewards program or not. These rates would vary from 1.69% - 4% plus the authorization fee of $0.10 - $0.25.
Yes, providing the card holder is notified before the transaction takes place.
The costs involved in small businesses accepting credit cards are quite high. Small businesses are charged a fee per credit card transaction. This fee will vary from country to country. It is common among most - if not all - major credit cards, that the transaction fee increases when a "premium" credit card is used. These "premium" cards allow the cardholder to reap rewards, while the business owner suffers. In Canada, for example, the transaction fee ranges from approximately 2% to 3% per transaction (for Visa and MasterCard).

Depends of the the Credit Card Processor as not all Merchant Account Providers charge this fee. The AVS fee (address verification service) is usually charged on a per transaction basis, usually immediately. Not all credit card processing companies charge this fee but some unscrupulous CC processing companies that do are not upfront about this fee and charge it in addition to the per transaction fee.
