It doesn't matter either way. I personally keep mine plugged in when I don't need to bring it anywhere. When you have it plugged, make sure the whole charger is unplugged from the outlet so it isn't still sucking up energy even when the laptop isn't plugged in.

No. check that the AC Power lead is plugged in and turned on. if the AC power lead is plugged in and turned on and still the battery is discharging, check that the AC power lead is working. this can be done with a multimeter. if the lead is faulty, replacements can be found. check the input power required by the laptop, as too much will blow the laptop up, and not enough will still make it discharge.
Every laptop is different. If you unplug your AC power adapter from the laptop and it start it up, then your battery is doing the work. If you want to check the opposite, remove the battery and make sure the AC power adapter is plugged in properly and start it up. If your laptop starts up from one form of power but not the other, either your connection is bad or the hardware itself (ex. battery or AC power adapter) is faulty.
If the Sansui MP3 player is not showing signs of charging when plugged into the laptop, perhaps the wrong cord is being used. There is a data cord and a charging cord. They look almost exactly the same. It is easier to charge this device by plugging it into a wall outlet, so that cords do not get mixed up.
This can reduce the life of the power adapter. The main problem with leaving the adaptor plugged into a laptop and power is it can shorten the battery life. This is especially true if your laptop has a Ni Cad battery. When you require the battery for 1 to 2 hours, it may only give you less than 15 minutes. While Lithium batteries are less prone to these problems, their life can still be reduced. If the laptop is going to be plugged in for extended periods of time, remove the battery.Once a month disconnect the laptop from the power and place the battery back into it.Run the laptop on the battery until you get a warning low battery beep.Connect the power back up to the laptop and let it charge overnight.Remove the battery the next day and leave it out until the next month or until you need to use your laptop without a power connection - whichever comes first.Follow this procedure and your battery should last 3 to 5 years. If you do not and leave your laptop connected to the power continuously, your battery will likely last 2 to 3 years at the most.Sources: General Knowledge
