My car wont start unless its in N and I half to have my foot on the gas till the rpms go up the it will idle on its own But when I put it in R or D it sometimes will die and or the rpms go up and down?

It sounds like you have a bad shifter cable. I had the same thing happing and i had it replaced and no proble since then. GOODLUCK
