If it is a conversion van then it may be air in the lines.

My G20 Conversion van had a problem with the heat. The AC worked great. I was told there was air in the radiator lines. This was difficult for me to believe however I jacked the front end of the van up and removed the radiator cap and then ran the van for about 20 min as per the online instructions. Of course nothing happened and I felt a little silly for trying.

A few weeks later I had to travel up highway 99 in California, remembering the trouble shooting ideas, despite the objections of the family I shut off the AC and ran the heater on high (no heat coming through the vents) About 5 to 6 minutes of traveling up hill, bouncing up and down on California's horrible roads at 70 miles an hour, low and behold the heat started poring out the vents, and has worked great ever since.
