The answer to this question is simple- there is NO way to get your PUK code without contacting your service provider. It goes hand-in-hand with the SIM card. If you try too many times and give the wrong answer, the SIM card will "Self-Destruct" and you'll have to buy another one (usually about 25-35 bucks).
Give them a call. It's the only way.
