This is a delicate question. If you remove smokers from being able to access public medical benefits, you then have to think "who else should be removed?". This could open up a Pandora's box. Should the public medical system help drug addicts, people who overeat, people who participate in dangerous activities such as skydiving or motorcross sports, people who drink alcohol and the list goes on.
