If we accept the premise that men take the initiative in all (even vaguely) romantic type relationships, then you would allow the guy to say hi first - although I find it hard to believe that this protocol is important enough that a guy would actually take offense that some mere girl is so self assertive that she actually said hi, without waiting for the guy to say hi first. But who knows, perhaps male chauvinism is making a comeback in the 21st century.
If he knew the girl was a guy from the start, you would have to say he was gay. He might have to re evaluate himself and be honest about it, but it is nothing that should be a problem. We love who we love for whatever reason and it is not for others to judge.

A guy doesn't have to have a girlfriend. If you want to that's ok. Nobody should have to force you or say that you should have a girlfriend. It's YOUR choice.but if you want to have sex than you should have a girl friend.

yes!most of the time the girl won't ask the guy out. its weird and the guy should always ask the girl out. But sometimes she could just be friendly towards you but not because she likes you. So if you want to avoid embarrassment, make sure she's into you first, but if you don't care, I say yes.
say "i like you" then whip out your hooded warrior
