The Space Shuttle Challenger disaster occurred on January 28, 1986, when Space Shuttle Challenger exploded 73 seconds into its flight after cold O-rings on one of the solid fuel rocket boosters failed. The booster then caught fire and resulted in the explosion of the boosters and external fuel tank. The Space Shuttle Columbia disaster occurred on February 1, 2003, when the Space Shuttle Columbia disintegrated over Palestine, Texas during re-entry into the Earth's atmosphere. This was shortly before it was scheduled to conclude its 28th mission, STS-107. Insulating foam fell from the fuel tank and struck the orbiter module during launch; this compromised the hull of the orbiter, and led to its sudden disintegration. In both disasters, all 7 crew members were killed and the shuttles utterly destroyed, with wreckage spread across a huge area. They were also more of a material failure, not a malfunction.

Challenger and Columbia.
challenger and Columbia
challenger, it exploded in 2002 when one of the rockets malfunccioned & challenger blewup
No space shuttles have exploded in space. Challenger disintegrated during launch and Columbia during the re-entry process. Neither were in space at the time. challenger and Columbia +++ The Challenger shuttle itself did not explode. The explosion was of the massive booster rockets lifting it into Space, after a seal failed on one, allowing fuel to leak and ignite. The shuttle remained fairly intact although very badly damaged, and I believe was subsequently recovered from the sea-bed.
Challenger exploded in 1986. Columbia in 2003.

Two space shuttles exploded; one upon take off and one on re-entry. The Space Shuttle Challenger exploded in 1986, 1 minute 13 seconds after take-off killing all seven members on board. The Space Shuttle Columbia exploded on re-entry in 2003 also killing all seven members on board.
