I'm assuming you have a tank, heater, thermometer, filter etc because they are the basic requirements for keeping fish. Your fish will all die if you don't look after their water conditions properly. I would do a heap of water changes and to kill the white spot (Ick) I'd raise their temperature to around 85F-87F for a few weeks.

You're possibly trying to mix two ingredients which don't have matching characteristics, or, which have them, but you don't have high enough alchemy skill to unlock. Each ingredient in the potion/poison must share a characteristic with at least one other. So one with Restore Health, one with Restore Health and Restore Stamina, and one with Restore Stamina would work fine. Two with Restore Health and one with Restore Stamina, would not.
