Let the clutch out until you feel it engage, then let it out the rest of the way. It won't jump. Practice makes perfect

If the vehicle is stopped without using the clutch , the engine will give jerks and turn off if the vehicle is in higher gear and the speed is not according to the range of the gear. If the problem is only happens when you take off then I would think it is clutch related; Oil on a clutch will cause it to grab,chatter etc. Also, if the clutch has a lot of miles on it, it may be just worn out.If this jerking happens as you drive the vehicle during acceleration , then your vehicle has some tune-up issues such as spark plugs, spark plug wires, cap,rotor.
your either lugging the engine or the clutch is starting to slip. try a little high rpm and be sure to remove foot completely from clutch in one fluid motion.

It depends on how you drive. If you shift smoothly, it should last you a while, at least 100k miles. If you are not smooth, i.e. the car jerks a lot and you "burn" the clutch more often by downshifting when you're at high rpms for example, then you can burn out the clutch in less then a year.
