As humid air rises higher and higher, to pass over places of high altitude, it cools and condenses, eventually falling as rain. Due to it rising faster, it condenses faster than over lower altitudes.

When mountains are involved: The mountains acts as barriers to humid air, as the air must rise quickly to pass over them. The air cools and condenses before passing completely over them, condensing, forming clouds, and falling as rain (but mostly snow) :)
