There are be a number of possible reasons:

1. Electrical failure due to a device going bad or

2. A short circuit on the printed circuit board causing PCB and or component damage

3. Drive's motor gone bad

4. Read/Write head crashed due to mechanical shock

5. Firmware of one or more chips on the PCB got courrupted Before concluding that the harddisk is bad you can also check if the jumper settings are correct, power supply voltage coming to disk are correct, the motherboard is correct and the cable connecting motherboard and harddisk is correct. - Neeraj Sharma

There are many causes of a hard drive disk failure. The biggest reason this would happen is if the computer had a virus. This would cause the hard drive to fail.

Either the hard disc is damaged or the installation of Mac OS X is damaged. Either way boot from the Mac OS X installation DVD. Insert the disc restart the computer and hold down the C key while it starts up. Once you have the desk top loaded select Disk Utility from the menu and run repair/first aid to try and fix the hard disc. If the hard disc is not too badly damaged you will need to reinstall Mac OS X. If you do not have back ups of the contents of your hard disc you will need to try and salvage what you can. Performing an Archive and Install will save your stuff if there is enough space on the hard disc.
