They are very similar. These two terms are usually used in the following way:
A product marketed as an "external hard drive" is an enclosed, stand-alone hard drive that is meant to sit on or by a computer desk. It is big & heavy enough that you wouldn't want to carry it around all the time.
A product marketed as a "portable hard drive" is an external hard drive that is small & light enough to be conveniently carried around in a computer bag, sometimes even in your pocket.
