The University of Glasgow in 1703 graduated the first medical doctor. Before that, the practice of medicine was by practice and was limited to the upper classes. There was limited literature available for study. Elizabeth Blackwell was the first woman doctor in the US - she graduated from Boston University in 1864. All in all, health care careers have been a product of the last two hundred years.
Health care is two words, however when used a modifier (healthcare provider) it's one word or used with a hyphen.

They are two different meanings. Health care is more general in meaning all the different ways health care can be implemented, including things like education and awareness. When it is combined into healthcare it means more specifically the facilities and programs that are put in place to help with health care, or care of health.
I think that there is a relation between the two. because ,imagine your self in a health care which the service on it poor what is your feeling about your health? and imagine the opposite!
It is actually both, usage is based on preference. Generally "healthcare" is used by hospitals, while "health care" is used by other services.
