Report her to your local law enforcement agency, social services gency or whatever agency of your state government charged with the child safety enforcement. However, be prepared to be able to produce proof of your allegation or you may be liable for filing false reports.

This would be very unlikely. If the mother is providing for the child and the child is not being abused why would the child be taken away from the mother. Why would the child be taken away from where it is being cared for properly and give to someone who has had problems with the law?

The reason why any mother would do drugs during or after her pregnancy is because they are extremely selfish. They dont love and care about their child enough to stop either. They dont worry about how emotionally scarred the child will become once the kid finds out (and they eventually do find out) that their mother is spending money on drugs when it should be spent on other things or that their mother is ruining her health and well-being just to have a good time. The mother is so selfish and messed up to worry about their kid not being able to fully trust anyone ever again. All because the mom is too worried about getting high. Its a shame.

You will have to have proof that she is using drugs in order to use that in a court hearing to try to get custody of your child. Her living with her mother does not qualify as being an unfit parent and if the child is 23 years old then they are an adult so there should be no reason to need to prove your ex wife is an unfit mother.

If a mother can stay clean from drugs for a year or more; have a decent job; live in a decent environment and provide for a child properly then she can go to court to at least go for joint custody to prove she is a fit mother. If she does win then she has to be drug free; no wild parties in the home with old friends who do take drugs; keep working and be sure her child is in safe hands at all times.
