Incomplete production or forming of normal twins. this also explains supernumerary appendages such as a Girl with Three arms I saw who worked in a department store. The third arm was short and stubby, but she had some control over it. a store in the same chain also had a camera vendor with only One arm, the shirt sleeve being tied back, it was strange.
