What contibutes to the high cost of medical care in America?

In my opinion the biggest issue is lack of price transparency. Medical care is just about the only service that you can not price shop. Clearly in an emergency you get the care ASAP. However, in most cases you have no idea what any procedure is going to cost until AFTER services are rendered.
You can call and ask what a facility charges for a procedure and you MAY get an answer, but they will almost never tell you what the insurance negotiated prices are and what you will have to pay after the insurance has paid their portion. They consider that propriatary and confidential contractual information.
