What could be wrong Silhouette blower only works on settings 4 5 only?

most likely it is the heater fan motor resistor. It is located under the dash on passenger side in heater box near blower motor. It is a common problem on all G.M vehicles. It is available at most aftermarket auto suppliers as well as G.M dealers.
