I've never seen it myself, but if you put in new water without leaving the water out to "age" for a few hours, there is a chance that your fish will suffer as the nitrogen bubbles form. Ever leave a glass of water out overnight? Those bubbles can even form INSIDE your fish which could cause the death you speak of.
