Residency training is for doctors after they graduate medical school so that they can learn a particular specialty. When a doctor first graduates from medical school, they have studied all of the major fields of medicine, but are not experts in any one particular field. For instance, they know as much about pediatrics as they do gynecology. But if they want to become a pediatrician, they need to become an expert in that field.

Different residencies are different in length. Most pediatrics residencies are 3 years in length, but some are 4, and some are 3 years for most residents and an additional 4th year if you want to be a "chief resident." People may pick this 4 year path if they want to further specialize, for instance if they want to do a fellowship in cardiology to become a pediatric cardiologist.
