"de bon coeur" means willingly or heartily. i don't think "bon des coeur" is actually a popular phrase.
En forme de coeur means 'heart-shaped' in French.
"Mon coeur" means my heart. "Rein" means "kidney" (probably a mispelling of "Rien" meaning "nothing"). So "mon coeur et rien de plus" would read "my heart and nothing more" in English.
