The external genitalia of the cat will look the same after it is fixed (or "spayed", some call it). When a female cat gets fixed, the veterinarian performs a surgery called an ovariohysterectomy where she/he will remove the ovaries and uterus of the cat, preventing her from getting pregnant or going into heat.

If you would like to breed cats, then don't get them neutered/spayed! If you do not get your cat neutered, it doesn't mean that it's going to get female cats pregnant, you'll just have to keep a close eye on them. I have noticed that when a cat is in heat, it gets rather annoying! But that's just me! :)

Neutering or spaying, doesn't matter the animal, will calm them down. The unspayed female will go into heat a number of times per year. They will howl for a mate during the night and keep you up. No matter what animal, they should be fixed so you don't have to go through the agony of watching them be in heat. Also, it's better for the animal to be fixed. I know with female rabbits they can get an ovarian cancer just like us if they're not fixed. Neutered males act very calm and slick, they become more attached to their owner than if they weren't neutered.
