Grading is based on the measuring of the tumor's cell appearance and the extent of it progress in the patient's body. The scale used is 1 to 4 (low grade 1, to high grade 4) the higher the grade or scale number the more aggressive the tumor.
The scale of malignancy is based on the tumor/cancer's pathology, and the variation in its cells to that of normal body cells. Medical labs look at the size or area of the tumor/cancer and its growth rate (mitotic index). They take into consideration the type of tumor/cancer and its location. They look at margin surrounding the tumor/cancer, to determine if it contains any cells that have strayed beyond the surgical site. They look for other cell abnormalities, which can indicate a change in cell structure which often indicates that they can become malignant and therefore may required chemotherapy or radiation therapy. They use this information and more, before they determine the degree of malignancy and thereby assigning a stage or grade to it. This is all done to determine the type of treatment which will be needed, if treatment is possible.
