It means that you fit into the society accepted theory of binary gender roles: there is only male and female and no blend, no spectrum. In general it means you have no question about where you belong when it comes to social rules of etiquette, family and sex roles, and you don't perceive anyone else as being anything other than 1 of the two binary genders.
Note that this is about perception, not truth. The "average" person believes that they are 'only' male or 'only' female when in truth everybody has grey areas of gender behavior: some women are aggressive, this is generally labeled as a male trait; some men are submissive and this is generally labeled as a female trait. Men nowadays take care of the children more often than their fathers and grandfathers did: this being a 'female' trait means that they are expressing (what appears to be) female behavior, and yet their physical sex (what's between their legs) hasn't changed, only the role has.
