50% water to 50% coolant is the standard unless you live in a very cold climate where it goes down to -25 degrees F or lower, then you may want to mix 25% water to 75% coolant but never straight coolant.
To flush the coolant in a 2002 Toyota Echo you will need to drain it first. Flush with fresh water and then refill with coolant and distilled water.
My 2002 manual says: 11.6 quarts for the Vulcan engine and 10.6 for the Duratec.
For a 2002 Ford Taurus : The thread on cap on the engine coolant reservoir is the pressure cap / radiator cap ( I believe the Motorcraft part number is RS-103 ) it's a 16 PSI cap
There is an anti-freeze/coolant reservoir on the left side of the engine compartment.Use the level markings on the side of the plastic to check the coolant levels.The Owners Manual covers this, including illustrations in some detail.

Plug at bottom or rear lower section of radiator
There is a coolant level sensor in the bottom of the coolant tank. If the tank has enough coolant, then the sensor is bad. You can buy a new tank for about $50 (Rockauto.com or Autozone) and replace it in an hour or so. Or just disconnect the cable from the sensor and check your coolant level on a regular basis.
