I would imagine it is a matter of preference and situation. For example, if you were to give a speech, the question usually comes up, "how would you liked to be addressed?" Thus, I guess it depends on the audience, the environment you find yourself in, and your personal preference. Your a reverend who holds a doctorate, and a doctor who is a reverend.
