I would imagine it is a matter of preference and situation. For example, if you were to give a speech, the question usually comes up, "how would you liked to be addressed?" Thus, I guess it depends on the audience, the environment you find yourself in, and your personal preference. Your a reverend who holds a doctorate, and a Doctor Who is a reverend. What is more important to you, that people see you as a reverend or doctor? Search your heart (you know what I mean) and I am sure you will come up with the right answer for yourself.
