A compound fracture is generally spoken of as a broken bone that has been shifted enough that part of the bone has broken through the skin and is "sticking out" to be visible. Kinda yucky, but that description is the key to diagnosis. It's hard to miss.
There are two issues to balance when aiding a victim of a compound fracture. One is bleeding, and the other is prevention of further tissue damage by bone fragments. You'll need serious help, so get that 911 call made ASAP and tend to your victim.
It is important to stop bleeding, but do not apply pressure to the fracture to stem bleeding. This may easily cause further tissue damage by moving the bone fragments around. Using a clean cloth, apply pressure near the wound (not on it!) to see if there is a point where you significantly slow bleeding without pressing on the wound itself. You'll have to use your best judgment in this. Do not let your victim bleed to death while waiting for emergency services to respond.
Do not move the victim unless absolutely necessary! This will minimize further tissue damage from the bone fragments. You have to immobilize the affected area if there is any way to do so. Cardboard, rolled up newspaper, sticks or what have you can be applied to this along with torn strips of clothing.
There is some good first aid advice in the Wilderness Manuals. Need a link? You got it.
First aid is an open compound word.
cast or split
setting it
A compound fracture (also known as an open fracture) is one which the bone protrudes the skin. The danger here is infection so you need to cover the wound to prevent infection, immobilise the area and call for help. There is not much else you can do in a first aid capacity until an ambulance arrives. When they do arrive they may decide to straighten the limb to help circulation and prevent bones fusing in the wrong position and they will be taken to hospital for an xray and further treatment.
