In the Medieval period the structure was very different, we now have a constitutional monarchy. The Queen is head of state but holds no power, up until the mid 17th century the monarch was an absolute monarch - they had complete control of the affairs of state. They had a council, chosen by the monarch who would advise and action, but it was the monarch who had the overall power and deciding vote so to speak - (very regularly members of the council would find themselves on the block - Thomas Cromwell being a good example)
