Sauron was one of the lieutenants of Morgoth back in the First Age, and he existed before Middle-earth..
My guess is that he would have left the cave, but one of Sauron's minons would have taken the ring and given it back to Sauron.

If you're referring to the guard who went out to meet them before the battle at the Black Gate, then that is the Mouth of Sauron, a Black Numenorean who is loyal to Sauron and is approximately 3,200 years old. He serves as the messenger of Sauron and is also considered the Lieutenant of the army of Mordor.
