All we know is that she was killed in the line of duty before Voyager ever made it back to Earth. However, the marriage and her death both occurred in an alternate timeline, which was wiped out due to interference from a future-Janeway who went back in time to bring Voyager home in the seventh year of their journey. (This coincided with the end of the series, and all of these events were described in the season finale, "Endgame.")
