I am assuming that you are talking about a Hematoma which is a build up of blood just below the skin on the outside of the dog's ear flap. This is usually caused by a trauma to the ear flap, such as shaking their head and hitting it on something. The only way this can be treated is to take your Dog in to be checked by your Veterinarian. He will sedate the dog and make several incisions to the ear flap in order to drain out the blood, then suture it back flat. This is a very simplified version.

Getting water into a dogs ears is the same as getting water into your ears, the dog should be able to get it out by shaking its head, but if there is some damage in the ear and the water is dirty, the ear could become infected. Never deliberately pour water into a dog's ears, if you have to clean them do this with a damp cloth. If there is a problem with you dogs ears take to to see a Vet.

No, swimmers ear can not switch ears. When you are in the water for a while you will notice your fingers get wrinkly. This is what happens in your ear after some time. When this happens it allows germs to get into your skin and causes an infection. So if one ear is completely fine, it will not get swimmers ear unless you allow water to be in your ear for some time.
