"The deputy Prime Minister immediately takes on the duties of the PM until the monarch invites someone to be the new Prime Minister."
This is incorrect if you are referring to the UK office of the Prime Minister. The Deputy Prime Minister is an honorary post which carries no responsibilities or right of succession. If the British Prime Minister died in office or was otherwise removed, the majority party would have to elect a new leader and then present him or her for approval to the Monarch.
Another Prime Minister would be appointed. The Head of State of the UK is the Monarch (Currently Queen Elizabeth II) and the PM is the head of the ruling Parliamentary Party. The Prime Minister becomes this as a result of a General Election, or as a result of the resignation of a sitting PM, if a PM were to die in office this would not necessarily mean a General Election would be called.

There has never been a Prime Minister of England.
England is a part of, but not the same as, The United Kingdom of Great Britain and Northern Ireland
I think the prime minister, on the event of resignation or death is indicated by someone of the government. In United Kingdom, it would be Her Majesty, the Queen, if I am not wrong. The line of succession depends on which country you're referring to.
When he dies, loses government, or resigns.
If the Prime Minister dies, the governing party selects a replacement. In some countries this person must then be approved by a monarch or president.

Pretty specific question... If a Prime Minister dies in office, or becomes otherwise incapable of performing his duties, the Minister specified by the most recent Acting Ministers Minute will become the acting Prime Minister. If the Prime Minister is unable to resume his duties, the governing party will select a new leader who will be appointed as Prime Minister by the Governor General.
I'd say its the vice-prime minister.** Edit:Canadians don't call it a "vice-prime minister". The Deputy Prime Minister of Canada is a position in the Canadian government. The position, which is not officially mandated by the Constitution of Canada, is actually a relatively weak one in cabinet. When the prime minster is out of the country it is accepted that no major decisions will be made. Without a portfolio or a ministry, the deputy prime minister is far less powerful than individuals such as the finance minister. Because of this the deputy prime minister is often given other cabinet responsibilities.Unlike the Vice-President of the United States, the Deputy Prime Minister does not assume the Prime Minister's office if he dies or resigns. The Governor General assumes the duties until he or she appoints a new Prime Minister in consultation with the Cabinet.Wrong, the Governor-General appoints the Finance Minister to temporarily cease power until a new Prime Minister is elected.
