If you're talking about Elder Scrolls IV: Oblivion, then you will get visited by the Wrath of Sithis at night, and no one will see him. Like other spirits, you have to use silver, daedric, or magic weapons to hurt him, and you can't go back into the sanctuary without being attacked until you kill him. In The Elder Scrolls V: Skyrim, there are no tenets, so you don't have to worry about that. You have 3 chances to get back in the DB, and if you break the tenets again you're permanently kicked out.

The Dark Brotherhood Sanctuary is located in the Cheydinhal East Gate.
The Dark Brotherhood is simply a side-quest in the game. It isn't crucial to the storyline but being in the Dark Brotherhood allows you to get weapons/spells/etc. that are normally unattainable.
