The theme music is commonly played on an instrument called a Theremin. It is an electronic instrument, and playing it doesn't involve any contact. The player puts their hands between two antennas, and moving one hand controls the pitch, the other controls the volume.
UPDATE EDIT (09/02/2014): If we were to look at the question, it asks what instrument(s) were used in the theme by Jim Parker. Clearly, Celia Sheen's performance on the Theremin as solo line stands out. But, I hear clarinets, violins, a cello, a piano, bass, Flute, and French horns (I believe). Jim Parker is an A-list master composer. Listen closely to his music, and you will find a gem in every music cue of every episode. I hope this better answers the question.
Different versions exist for the Doctor Who theme song, although the basic melody remained the same. The original (1963), as well as many later versions through 1980 that used the original recordings, were electronic music made with primitive sampling methods and oscillators. After season 17, synthesisers were used.
