When you are talking about fish, ( e.g. catfish and carp) it refers to the fact that they feed off what has settled on the lake or stream bed. This has been said to make their flesh not as tasty to humans. It also makes their flesh higher in metals and chemicals that can be bad for us, so it is not good to eat too much of them - unless they are farm raised. When you are referring to a person, it is a derogatory epitaph indicating the person is disgusting.

The term "bottom feeder" (or benthos) is used to describe fish that live and feed off of the lowest levels of their aquatic environment.

They generally have many adaptations that help them do so, like mouths that face downwards, flatter bodies, and long barbels to feel the ground. Bottom feeders are also used in aquariums to clean up uneaten food and waste, sometimes eating algae too. They can also live more peacefully with otherwise aggressive fish that prefer to dwell near the top of the tank.
