Just had this problem with my 1999 tacoma 3.4, and had the CO2 level in the radiator checked. Showing low levels of Carbon Dioxide indicating that the head gaskets are bad. My truck also has been getting a little bit of a hesitation when at idle. The shops are telling me I need to get it fixed ASAP or I will be putting a new motor in it. Bottom line, have the CO2 levels checked to test the head gasket. Hope this helps.
Can be a hole in the radiator or a problem with one or more of the hoses. A mechanic can run a pressure test to see where the problem(s) are located at.

If a vehicle is loosing coolant then there is a leak. If no leak is visible then the coolant could be leaking into the oil, or leaking into a cylinder and being blown out through the exhaust.
it sounds like your coolant over flow tank is leaking. it is a common problem with these cars. might also check your radiator for leaks or your exhaust for coolant, but if there is coolant in your exhaust that would mean blown heads

You should able to tell if it's leaking so I assume that you are not seeing any coolant where you park so it's a good chance you got a blown head gasket and the coolant is leaking in your cylinders and being burned off or leaking into your oil. Check your oil after running and if it is foamy it has coolant in it.

I would check the hoses leading into your radiator, and also the overflow for antifreeze. If that is not the problem, it could be your heater core. Your heater core might be leaking, you might have a hose leaking
