That depends on who you are speaking to. For instance if you are talking to your doctor you would describe a problem. Most often with strangers, you might say, I am fine, thank you; How are you?

You just tell her. Don't make a big deal out of it, but if you just mention it to her, she will probably be embarrassed, but she should get over it. You can warn her that you don't want her to be embarrassed and if the problem was reversed you would want her to tell you and then just tell her. You can tell her you are worried about her health if you need another excuse.
