It depends on what is causing the pain. I have RA and this can cause inflammation which is painful For this I find that cold for a short time can help. However as a result of theinflammation I also have problems similar to osteoarthritis and in that situation there is nothing better then letting hot water run offer the affected joint.

If you have inflammation then it can be help-full to alternate between heat and cold. You have to sort it out for yourself how long the periods will be About 5 min hot then 5 cold and so on until you think you are getting some where with the treatment. In the case of RA a bit of cold often helps.
If the swelling is due to trauma such as a punch to the jaw, ice is best. If swelling is inside the mouth, an abscess is the most likely cause and heat is best as it draws the pus to the surface where it will burst and the relief will be instant.
