An ileoanal anastomosis is a procedure performed on patients who have not responded to more conservative treatments. The small intestine is composed of three major sections: the duodenum, which is the upper portion into which the stomach empties; the jejunum, which is the middle portion; and the ileum. The ileum is the last portion of the small intestine and empties into the large intestine. The large intestine is composed of the colon, where stool is formed, and the rectum, which empties to the outside of the body through the anal canal. Surgical removal of the bowel is usually a procedure of last resort for a patient who has not responded to less invasive therapies.Many patients with ulcerative colitis, an inflammatory condition of the colon and rectum, can be treated by medications or dietary changes that control the symptoms of the disease. For patients who fail to respond to these approaches, however, the creation of an ileoanal anastomosis removes most or all of the diseased tissue. Certain types of colon cancer and a condition called familial adenomatous polyposis, or FAP, in which the inner lining of the colon becomes covered with abnormal growths, may also be treated with ileoanal anastomosis. Post operative following surgery, the staff will instruct the patient in the care of the stoma, placement of the ileostomy bag, and necessary changes regarding diet and lifestyle. The creation of an internal pouch allows the individual to regain health and a normal lifestyle without a permanent ileostomy. After the operation it takes a few days before peristalsis (bowel action) resumes and you will be given nothing to eat or drink. Nourishment will be given by intravenous. and a gastric tube will remove bile and acids from the stomach. Once activity resumes in the bowel you will begin to pass gas and small liquid stools into the appliance attached to your abdomen. The gastric tube will be removed and your doctor will allow you to have fluids by mouth.. Why a Diet? 1. Reduce excessive bowel stimulation. 2. Produce adequate nutrition Procedure.

1. Chew everything thoroughly. 2. Introduce new foods gradually this means try only one new food at a time. if a problem arises the cause is easily identified and may be deleted from your diet. Please Note, this doesn't mean that you will never be able to eat that food, only that you should wait a while before trying that food again in the future. The following is a list of foods to be considered when preparing diets for an ileoanal/ileostomy patient. 1. Foods high in roughage may cause blockage, introduce these foods to your diet gradually: bran, nuts, dried fruits and raisins, seeds, skins, beans, pineapple, popcorn, asparagus, Oranges, shrimp, kernel corn, raw vegetables, Chinese food, celery, coleslaw, salad, mushrooms, lentils, tomatoes 2. Foods that increase output: raw fruits/vegetables, chocolate, leafy green vegetables, caffeinated beverages, spicy foods, carbonated beverages 3. Foods that slow output/increase transit time: bananas, applesauce, tapioca, peanut butter, cheese, boiled rice, marshmallows 4. Foods that may cause skin irritation: Any foods with seeds, celery, Chinese/oriental vegetables, coconut, nuts, spicy foods, dried fruits (raisins, figs, etc.), certain raw fruits & vegetables (oranges, apples, coleslaw & corn).

The diet of an ileostomy patient will depend largely on the type of ileostomy performed, whether continent or incontinent. For around a month after surgery, a diet low in roughage is suggested to give the surgery time to heal. Eat at regular times, avoid overeating, and chew food well to prevent gas. After healing, a regular diet can be resumed, adding foods a few at a time to rule out if one thing is causing problems.

a low-residue diet for several days prior to surgery. A liquid diet may be ordered for at least the day before surgery, with nothing by mouth after midnight.
